What will a Support Coordinator do for me ?

Your support coordinator will  complete referrals and organise services for you . Support Coordination is a Capacity Building support, and part of the role is to ensure you learn the skills to be able to do this for yourself over time .

 

What if I am not happy with my current NDIS plan?

Your support coordinator  will assist you to contact NDIS to review decisions or your plan. Your support coordinator will assist you to gather information and reports if needed .

 

Can I change to a different Support Coordinator  provider  if I am not happy ?

Yes, You  have choice and control  of your plan , budget and services. Total care Team currently have  6 Support Coordinators available for you to choose. There is flexibility to change within Total Care Team or else to another provider. You will need to give notice of your intention to change providers so your support coordinator can assist you to find another provider, complete a handover report and a referral .

 

Who is Total Care Team?

Total Care Team commenced when Sussanne identified the need for private and local Support Coordinators , Occupational Therapists, Therapy Assistants and Social Workers who can provide timely, friendly, professional support to assist  NDIS participants to reach their goals.

 

How will an NDIS plan benefit me?

If you are deemed eligible, an NDIS plan will be developed with you. Based on this plan, you will receive funding on an annual basis to purchase the services, aids and equipment you need. As your circumstances change, you can apply to have your plan reviewed . The NDIS is not means tested and the amount of support you receive from the NDIS is based on your specific needs, not on your income and assets.
